I hereby release
Academy of Ballet & Dance Arts, Woodlands Dance Academy, its agents and employees, from all liability for personal
injury, illness or property damage whether or not caused by the
negligence of The Academy, its agents or employees. I understand
that I am responsible for all fees and all tuition payments throughout
the dance year of August through June. I understand that all fees are
non-refundable. I certify that I am in good health and capable of
participating in all school activities and classes. I hereby give
permission for The Academy to take and use photographs for promotional
uses for the school. I understand that I am
waiving any claims, liens, demands or causes of action which I may now,
or in the future, possess against The Academy arising out of or in any
way related to my participation. |
